For decades, we have been trapped in a self-constructed "Paper Reality"—a world obsessed with ticking boxes, chasing impossible Zero Accident targets, generating excessive Paperwork, and assigning Blame. This reality looks pristine in the boardroom, yet holds zero connection to the complex, messy, and human operational truth of the shop floor.
