Investigate their "Success" with the same rigor you would investigate a "Failure." Ask: "How did you go this fast? What controls did you bypass? What margins did you erode to get here?" If you reward the deviation based on the positive outcome, you are actively funding the next catastrophe. You must reward adherence to the standard, even when it costs short-term speed.
2. Worship the "Weak Signals" (Preoccupation with Failure)
In a standard company, a "Near Miss" is good news. "Phew! We almost had an accident, but we didn't. Back to work." In an HRO, a "Near Miss" is a crisis. It is a Free Lesson that must be exploited aggressively. It reveals that the system's primary defenses failed, and only luck saved you. HROs obsess over weak signals—a small leak, a loose bolt, a strange vibration, a minor process deviation—because they know these are the precursors, the early warning radar, for the explosion. They incentivize reporting them.
3. The "Stop Work" Authority (Radical Psychological Safety)
You must create a culture where the lowest-ranking employee has the absolute power—and the duty—to stop the highest-ranking manager without fear of retribution. If a junior engineer says, "I'm not comfortable with this O-ring data," the Senior Manager must say "Thank you for speaking up. Let's stop and look," not "Put on your management hat." This requires radical Psychological Safety, as defined by Amy Edmondson. If you shoot the messenger just once, you blind the entire organization forever. No one will ever speak up again.
4. Audit "Work-as-Done," Not "Work-as-Imagined" (Gemba Walks)
Auditors usually sit in air-conditioned offices checking paperwork, verifying that procedures exist (Work-as-Imagined). This is useless for finding deviance. To find deviance, leaders must go to the Gemba (the Japanese term for "the real place"—the shop floor, the operating room, the construction site). How to do a Gemba Walk for Deviance:
Don't talk, just watch.
Look for homemade tools, duct tape, handwritten notes on machines (signs of workarounds).
Ask workers: "What is the hardest part of following this procedure?"
Ask: "When was the last time you had to bend a rule to get the job done?" If you find a deviation, ask the worker "Why does doing it this way make sense to you?" instead of punishing them. You will likely find that the system is broken, and their deviation is a rational fix.
5. The Pre-Mortem and Red Teaming
Pre-Mortem: Before a major project or launch, gather the team. Assume the project has already failed spectacularly. Now, work backward to generate plausible reasons why it failed. This breaks the optimism bias and allows people to voice concerns they were suppressing.
Red Teaming: Form independent groups whose explicit job is to aggressively challenge the prevailing assumptions, attack the plan, find its weak points, and act as the "Devil's Advocate." This breaks Groupthink.
The Bottom Line: The Eternal Vigilance
The most dangerous phrase in the history of human organization is: "We’ve always done it this way, and it’s been fine." Just because you haven't had an accident yet, doesn't mean you are safe. It just means the probability curve hasn't caught up with you yet. You are driving on icy roads without a seatbelt, and you think you are safe because you haven't crashed... yet.
Normalization of Deviance is like rust on a steel bridge. It is natural. It never sleeps. It eats away at your standards slowly, quietly, relentlessly, night and day, until the structure collapses under a normal load. As a leader, you have two choices, and only two:
Actively, exhaustively fight for your standards every single day, injecting energy into the system and creating a culture of uneasiness and discipline.
Or accept the drift, enjoy the temporary efficiency and higher margins, and wait for the funeral.
Don't let "Normal" become "Deadly."
