Rewrite the corporate investigation procedure to ban the bare phrase "Retrain the Worker" as a corrective action. Replace it with a requirement for a "Systemic Competency Review."

If an investigation team wants to recommend training, they must prove with evidence that the worker genuinely did not know how to do the job (a knowledge deficit). If the worker knew how to do it, and still failed (a slip, lapse, or rule-based error under stress), training is forbidden as a corrective action because it does not address the cause. The team must find out why a competent, experienced person failed in that context.

Solution 4: Engineer the Budget (CapEx Authority for Safety)

Safety independence requires financial independence. Give the Safety Department a dedicated CapEx Budget, not just an OpEx budget for training materials.

If the Safety Director has to beg Operations Directors for money from their production budget to fix a machine guard, safety will always lose to production targets. If the Safety Director has their own dedicated, board-approved fund for "Safety Critical Hardware Improvements" that does not require Operations approval, the Hammer becomes less necessary, and engineering solutions become viable.


Conclusion: Stop Hammering the Screws

We cannot train our way out of bad design. We cannot procedure our way out of the laws of physics. We cannot discipline our way out of the realities of human biology and cognitive limitations.

As long as we treat every safety problem as a "Behavioral Issue" or a "Training Deficit," we will continue to hurt people, destroy value, and stall progress. We are applying a medieval remedy (lectures, scolding, and parchment rules) to modern technological complexity.

We must recognize a fundamental truth: The worker is not a nail to be hammered into submission to a flawed system. The worker is the canary in the coal mine. When they make an error, they are revealing a flaw in your architecture, your processes, or your resources. When the canary dies, you don't train the next canary to breathe less oxygen. You fix the ventilation system.

It is time to retire Maslow's Hammer. Throw away the hammer. Pick up the blueprint.
