It is a pristine, sanitized PDF document. It has neatly aligned bullet points. It has a linear, chronological timeline calculated down to the second. If your investigation report cannot explain why the "unsafe" action made rational sense to the worker in that context, you have not understood the accident. You have just written a piece of fiction.

3. Use "Learning Teams," Not "Investigation Squads"

Don't send an "Investigator" (which sounds like a police detective) to interview "Suspects." This shuts down information flow.

Because you cannot fix a complex system if you are only reading the fairy tale version of how it works.

Stop looking for the Villain. Stop looking for the Root Cause. Start looking for the System.
