It is perhaps the most important, yet most ignored, mental model for any Safety Director, CEO, or Consultant entering a new organization:

There exists in such a case a certain institution or law; let us say, for the sake of simplicity, a fence or gate erected across a road. The more modern type of reformer goes gaily up to it and says, “I don’t see the use of this; let us clear it away.” To which the more intelligent type of reformer will do well to answer: “If you don’t see the use of it, I certainly won’t let you clear it away. Go away and think. Then, when you can come back and tell me that you do see the use of it, I may allow you to destroy it.”

In the modern QHSE (Quality, Health, Safety, Environment) world, we are currently in the grip of a reductionist fever. We are obsessed with "Decluttering," "Lean Safety," "Agile Compliance," and "Cutting Red Tape." New leaders arrive with a mandate to "modernize" and "streamline." They look at a 20-year-old rule, fail to see its immediate commercial logic, label it "bureaucracy," and delete it.

The Catastrophe: Often, that "useless" rule was a load-bearing wall disguised as clutter. It was a Regulatory T-Cell put there to prevent a very specific, rare, and catastrophic failure mode that hasn't happened in 15 years precisely because the rule exists. By removing the fence because they didn't see the bull, the reformer invites the bull back into the china shop.
